Isolated Right Ventricular Infarction without Right Ventricular Dysfunction: A Case Report

BACKGROUND: Myocardial infarction is the leading cause of occlusion coronary artery. Meanwhile, right ventricular myocardial (RVMI) usually associated with inferior left in 10–50% cases. We presented isolated RV infarcts which are rare cases that happen only 3% total infarction. CASE PRESENTATION: a case 67-year-old man sudden chest pain, shortness breath, and history diabetes mellitus. From his 12 lead electrocardiogram, there no specific ST elevation yet V3R V4R elevated troponin I highly sensitive value. The patient developed junctional bradycardia early percutaneous intervention backup temporary pacemaker showed proximal artery (RCA) CONCLUSION: RVMI commonly occurs dominant RCA also supplies sinoatrial node atrioventricular node. Therefore, due to its location passed through RCA, complete heart block may happen. Isolated (RV) because either marginal as predominant occluded, or non-dominant occluded by thrombus. impair contractility causing decrease stroke volume this condition leads hypotension, shock, severe hemodynamic derangement. acute occlusions do not result significant necrosis, one reasons collateral flow from system further protects dysfunctions our patient.
